NEW FOR 2020: Supplemental Life Insurance Enhancement.

Participants covered under our current group Life Insurance benefit through The Hartford can purchase additional coverage of up to $150,000 (in increments of $10,000), as well as coverage for their spouse and dependents.

This optional coverage is voluntary. Churches can choose to offer it to their staff and have the premiums paid by the church, or by the employee through payroll deductions. Adding the supplemental Life Insurance coverage for an employee, spouse, or dependent(s) will not require a physical exam or any medical screening questions. Church administrators can enroll and manage employee benefit coverage options through the EPC Benefits Portal. Administrators should contact the EPC Admin office at 877-578-8707 or to set up their user ID and password and for additional information. The EPC Benefits Portal is used to enroll and make changes to the plan options for eligible employees in the:

• Life/AD&D/Long-Term Disability (LTD) Bundled Benefit Plan.

Participants should use this form to enroll or make changes to their Benefit Plans coverage.

Church Billing Setup Form

Churches that are new to EPC Benefits or existing participating churches that need to update their billing information.

Church Benefit Election Form

This form must be completed by churches enrolling in EPC Benefits for the first time. Changes will be reflected in the online portal only after this form has been completed and submitted.

The Hartford Life/Disability Beneficiary Designation Form

This form must be submitted to the Admin Office for employees who enroll in the Life/Disability bundled benefit.

These forms are fillable PDF documents. You must download to your computer (when the document opens in your browser, right-click and “Save As” to your computer) and fill them out in Adobe Acrobat or the information will not be saved. The 2020 Benefits Guide provides a high-level overview of the plans, including eligibility requirements, five medical plan options, two dental options, vision benefits, life and disability benefits, retirement plan, telephonic medical care, and more.


The monthly premium rates for the 2020 Medical, Dental, Vision, Life and AD&D, and Long-Term Disability (LTD) plans are available at This online calculator is a great tool for budgeting and for choosing a plan that fits within your budget.

Also included are premium rates for our Missionary Gold Plan.


All of our medical plan benefit levels remain unchanged for 2020, with the exception of $5 reductions to Teledoc and Retail Clinic co-pays, and a co-pay increase to $175 for Emergency Room visits for all our PPO plans. The ER co-pay remains below national averages.

Benefit Resources, Inc., provides medical insurance coverage through Highmark Blue Cross Blue Shield. EPC Medical Plan participants have access to more than 96 percent of hospitals and 95 percent of doctors and specialists nationwide contracted through the Blue Cross Network. We offer three PPO plans and two high-deductible medical plan options for you to choose from. When you enroll in the Medical Plan, you will also be enrolled in the Prescription Drug Plan.

The BRI Prescription Drug Plan is administered by Express Scripts.

Prescriptions dispensed for acute care (short-term) medications and initial fills of maintenance (long-term) medications may be obtained through any retail pharmacy for up to a 30-day supply. Short-term drugs include antibiotics and other medications that you take for short periods of time. Long-term drugs, also called maintenance medications, are those you take on an ongoing basis, such as drugs that treat high blood pressure, cholesterol or chronic diseases. Maintenance medications are only available under the Smart90 program.

For those using Specialty Medications that require focused care and monitoring to maximize effectiveness of treatment, BRI has contracted with Accredo, an Express Scripts specialty pharmacy with more than 25 years of experience in the field. Accredo provides comprehensive local clinical services and 24/7 personalized support and education to patients navigating complex health issues through pharmaceutical therapy.

New for 2020: Dental Dental is the new provider, starting January 1, 2020.

BRI offers two dental plans through Delta Dental, and rates for both the Low Plan and High Plan are reduced from 2019 levels.

The Low Plan provides a lower benefit level for a lower monthly premium. The High Plan provides more comprehensive coverage with a higher annual maximum benefit. Participants can visit any licensed dentist, but using a Delta Dental Premier or PPO provider will result in the greatest out-of-pocket savings.

Delta Dental’s coverage includes:

Access to one of the largest networks of dentists nationwide, with more than 150,000 practicing dentists and 330,000 locations.
No-cost for preventive care.
Low Plan annual maximum of $500 (the total amount per person the plan will pay toward the cost of dental care each year).
High Plan annual maximum of $1,500 (the total amount per person the plan will pay toward the cost of dental care each year).
 SmileWay, which provides enhanced coverage for participants who have been diagnosed with diabetes, heart disease, HIV/AIDS, rheumatoid arthritis, or stroke. Opting into this program provides added benefits of 100% coverage for periodontal scaling, prophylaxis, or periodontal maintenance procedures. To learn more, see

Note that unless a current participant drops their Dental coverage during Open Enrollment, those who are enrolled in the EPC dental plan in 2019 will automatically be enrolled in the dental plan for 2020. New Delta Dental ID cards will be distributed in December, and also will be available to print online.

BRI offers a Vision Plan through the EyeMed network of providers. The vision plan covers an exam and lenses every 12 months, and covers eyeglass frames every 24 months.

For the greatest benefits, visit a doctor in the EyeMed network—you can search for a doctor online at EyeMed has several networks, so be sure to select the EyeMed “Access” network when searching.

BRI offers Life, Accidental Death & Dismemberment (AD&D), and Long-Term Disability (LTD) insurance through The Hartford. Rates and coverages are unchanged from 2019.

Life insurance coverage offers eligible employees a $50,000 Term Life Insurance policy (decreases after the age of 65). Should your death be due to an accident, your beneficiaries would receive an additional AD&D benefit equal to the amount of the life insurance benefit.

LTD insurance provides partial income replacement should you become disabled and unable to work. If you experience a disability that lasts 90 days or more, the LTD plan will replace 60% of your monthly pre-disability salary up to the plan maximum.

New for 2020! Elective Supplemental Life and AD&D Benefits.

These supplemental coverage options are available only to employees who are currently enrolled—or will be enrolled—in the EPC’s Life/AD&D/LTD Insurance bundled benefit in 2020.

Employees may elect to supplement their basic Life and Accidental Death & Dismemberment (AD&D) coverage with additional coverage through The Hartford. Employees may purchase—and are guaranteed to receive—Life and AD&D insurance coverage of up to five times their annual earnings, to a maximum of $150,000 coverage. Those enrolling during Open Enrollment and newly eligible employees will not need to undergo a medical exam or submit evidence of insurability to receive this coverage.

In addition to acquiring their own supplemental Life/AD&D coverage, our new benefit allows employees to purchase coverage for their spouse and dependent(s) under our group rates. As with the employee coverage, a medical exam and evidence of insurability will not be required if enrollment takes place during Open Enrollment, or if a new employee is enrolling for the first time.

Supplemental Life and AD&D Amounts

Employee: A maximum of $150,000 or 5x earnings, in increments of $10,000

Spouse: Up to 50% of the employee’s supplemental life amount in increments of $5,000 (to a maximum of $75,000)

Child(ren): $10,000


Participants enrolled in the EPC Benefit Plans—including the Retirement Plan—can take advantage of the Amplifon hearing aid discount program. This program provides discounted hearing aids through more than 5,000 audiologists throughout the country. There are other benefits provided when working through this network.

This benefit is available to EPC employees working 20 hours or more per week.

The EPC has negotiated with Colonial Life to make Life, Accident, Critical Illness, and Short-Term Disability Insurances available to EPC employees who are not participating in the Life and Disability benefits available through The Hartford. Full and part-time employees working only 20 or more hours per week are eligible, even if not enrolled in any EPC Medical or other benefit plan.

These are great coverages for those working part-time, or not eligible to receive the EPC’s regular Life and Disability Insurance benefits. These policies are issued directly to the employee by Colonial Life, so staff members may pursue enrollment at any time and are not restricted to Open Enrollment time limitations.

The BRI 403(b)(9) Retirement Plan offers you an opportunity to save for retirement with both pre-tax and post-tax dollars. The Plan is managed by Fidelity Investments and offers more than 22 mutual fund investment options in the standard lineup. Additionally, Fidelity BrokerageLink® provides plan participants with the opportunity to select from thousands of mutual funds and other investment options beyond the standard lineup, such as stocks, Exchange-Traded Products (ETPs), CDs, and more.

Your church may contribute to your 403(b)(9) account to help your savings grow. Employer contributions are deposited to your traditional pretax 403(b)(9) plan. Any amount the church contributes to your account is fully vested and yours to keep. Individuals age 50 and older also may make additional “catch up” contributions. Check with your church administrator for more information.

Note that enrollment in the 403(b)(9) Retirement Plan—and changes to investment allocations—can occur any time, and is not limited to the Open Enrollment period.
