The EPC provides two dental plans through Principal, and both allow you to visit any dentist of your choice. However, you receive the greatest benefit when you visit dentists in the Ameritas network.

The Vision Plan covers one exam and lenses every 12 months, and eyeglass frames every 24 months. Your vision plan uses the EyeMed network of providers. To get the greatest benefit, visit a doctor in the EyeMed network by searching online at www.eyemedvisioncare.com (select the EyeMed “Access” network).

HEARING AID PROGRAM
New for 2018: Participants enrolled in the EPC benefit plans, including the Retirement Plan, will have access to the Amplifon hearing aid program. This program provides discounted hearing aids through more than 5,000 audiologists throughout the country. There are other benefits provided when working through this network. Medical consultations through Teladoc are available for $10 to employees who are enrolled in the Platinum, Gold, or Silver plans. For employees enrolled in the Gold HSA plan, medical consultations through Teladoc are available for $40. For more information about Teladoc, see www.teladoc.com/videos.
The EPC is invested in your well-being. One way we do that is to support a wellness program—a systematic approach to healthcare that emphasizes employee health and disease prevention instead of the treatment of an illness. Studies show that employees who participate in such a program are:
• Absent from work less often.
• More engaged in managing their health.
• More productive.
• Less costly to the healthcare system.
The EPC Wellness Program is voluntary, has no cost to participate, and provides immediate and long-term benefits. To encourage participation, eligible individuals who complete an online Wellness Profile will receive a $50 gift certificate redeemable at one of more than 200 retailers.

After completion of your Wellness Profile, you will receive a confirmation email at the address you used to register at www.highmarkbcbs.org. You should receive your award by mail from Hallmark Business Connections by the 15th of the following month of completing your Wellness Profile. Eligible EPC members may receive only one reward per calendar year.
