NEW FOR 2020: Supplemental Life Insurance Enhancement.
• Participants covered under our current group Life Insurance benefit through The Hartford can purchase additional coverage of up to $150,000 (in increments of $10,000), as well as coverage for their spouse and dependents.
• This optional coverage is voluntary. Churches can choose to offer it to their staff and have the premiums paid by the church, or by the employee through payroll deductions. Adding the supplemental Life Insurance coverage for an employee, spouse, or dependent(s) will not require a physical exam or any medical screening questions. Church administrators can enroll and manage employee benefit coverage options through the EPC Benefits Portal. Administrators should contact the EPC Admin office at 877-578-8707 or EPC@cdsadmin.com to set up their user ID and password and for additional information. The EPC Benefits Portal is used to enroll and make changes to the plan options for eligible employees in the:
• Medical Plans.
• Dental Plans.
• Vision Plan.
• Life/AD&D/Long-Term Disability (LTD) Bundled Benefit Plan.

This form must be completed by churches enrolling in EPC Benefits for the first time. Changes will be reflected in the online portal only after this form has been completed and submitted.
These forms are fillable PDF documents. You must download to your computer (when the document opens in your browser, right-click and “Save As” to your computer) and fill them out in Adobe Acrobat or the information will not be saved. 2020 PREMIUM RATES CALCULATOR
The monthly premium rates for the 2020 Medical, Dental, Vision, Life and AD&D, and Long-Term Disability (LTD) plans are available at www.epc.org/benefits/2020premiumrates. This online calculator is a great tool for budgeting and for choosing a plan that fits within your budget.
98point6 provides primary care physician visits, on-demand 24/7, from any location via text messaging using your mobile phone or smart device. Visits with 98Point6 physicians are available to employees and dependents ages 1 and older enrolled in any of BRI’s Medical/Prescription Drug, Dental, Vision, or Life Insurance and Disability benefit plan. Each text-based visit is available at no cost to PPO plan participants, and for all other eligible participants there is only a $5 co-pay.
The EPC has already confirmed Plan participants’ eligibility for 98Point6. Simply download the app on your smart device and register using the name on any of your EPC Benefit Plan insurance cards. You will have immediate access to the service if a medical question or health condition arises.

2020 VISION PLAN
BRI offers a Vision Plan through the EyeMed network of providers. The vision plan covers an exam and lenses every 12 months, and covers eyeglass frames every 24 months.
For the greatest benefits, visit a doctor in the EyeMed network—you can search for a doctor online at www.eyemedvisioncare.com. EyeMed has several networks, so be sure to select the EyeMed “Access” network when searching.

2020 BASIC LIFE, AD&D, AND LONG-TERM DISABILITY INSURANCE
BRI offers Life, Accidental Death & Dismemberment (AD&D), and Long-Term Disability (LTD) insurance through The Hartford. Rates and coverages are unchanged from 2019.
Life insurance coverage offers eligible employees a $50,000 Term Life Insurance policy (decreases after the age of 65). Should your death be due to an accident, your beneficiaries would receive an additional AD&D benefit equal to the amount of the life insurance benefit.
LTD insurance provides partial income replacement should you become disabled and unable to work. If you experience a disability that lasts 90 days or more, the LTD plan will replace 60% of your monthly pre-disability salary up to the plan maximum.
For more information on Life, AD&D, and LTD insurance through The Hartford, please refer to the documents below. Please note that the LTD Plans are different for individuals who are Full-Time Salaried, Management Staff, Ordained Staff, and Full-Time Salaried earning more than $80,000 annually.

SUPPLEMENTAL LIFE AND AD&D INSURANCE
New for 2020! Elective Supplemental Life and AD&D Benefits.
These supplemental coverage options are available only to employees who also enroll in the EPC’s Life/AD&D/LTD Insurance bundled benefit in 2020.

SUPPLEMENTAL LIFE/AD&D INSURANCE
Employees may elect to supplement their basic Life and Accidental Death & Dismemberment (AD&D) coverage with additional coverage through The Hartford. Employees may purchase—and are guaranteed to receive—Life and AD&D insurance coverage of up to five times their annual earnings, to a maximum of $150,000 coverage.
Employees who acquire their own supplemental Life/AD&D coverage also can purchase coverage for their spouse and dependent(s) under our group rates.
Benefit reductions start at age 65; consult the Plan Highlights documents below for more information.

Employee: A maximum of $150,000 or 5x earnings, in increments of $10,000
Spouse: Up to 50% of the employee’s supplemental life amount in increments of $5,000 (to a maximum of $75,000)
HEARING AID DISCOUNT PROGRAM
The Amplifon hearing aid program provides discounted hearing aids through more than 5,000 audiologists throughout the country. There are other benefits provided when working through this network.
This benefit is available to EPC employees working 20 hours or more per week.

VOLUNTARY LIFE, ACCIDENT, ILLNESS, AND SHORT-TERM DISABILITY
The EPC has negotiated with Colonial Life to make Life, Accident, Critical Illness, and Short-Term Disability Insurances available to EPC employees who are not participating in the Life and Disability benefits available through The Hartford. Full and part-time employees working only 20 or more hours per week are eligible, even if not enrolled in any EPC Medical or other benefit plan.
These are great coverages for those working part-time, or not eligible to receive the EPC’s regular Life and Disability Insurance benefits. Enrollment can be initiated at any time and is not restricted to Open Enrollment time limitations.

403(b)(9) RETIREMENT PLAN
The BRI 403(b)(9) Retirement Plan offers you an opportunity to save for retirement with both pre-tax and post-tax dollars. The Plan is managed by Fidelity Investments and offers more than 22 mutual fund investment options in the standard lineup. Additionally, Fidelity BrokerageLink® provides plan participants with the opportunity to select from thousands of mutual funds and other investment options beyond the standard lineup, such as stocks, Exchange-Traded Products (ETPs), CDs, and more.
Your church may contribute to your 403(b)(9) account to help your savings grow. Employer contributions are deposited to your traditional pretax 403(b)(9) plan. Any amount the church contributes to your account is fully vested and yours to keep. Individuals age 50 and older also may make additional “catch up” contributions. Check with your church administrator for more information.
The BRI 403(b)(9) retirement plan ID number is 89560.
Note that enrollment in the 403(b)(9) Retirement Plan—and changes to investment allocations—can occur any time, and is not limited to the Open Enrollment period.
