Welcome to EPC Benefit Resources, Inc. We hope the information you find here will be helpful as you learn about the EPC Benefit Plans for the 2021 calendar year.

Church administrators can enroll and manage employee benefit coverage options through the EPC Benefits Portal. Administrators should contact the EPC Admin office at 877-578-8707 or to set up their user ID and password and for additional information. The EPC Benefits Portal is used to enroll and make changes to the plan options for eligible employees in the:

Medical Plans.
Dental Plans.
Vision Plan.
• Life/AD&D/Long-Term Disability (LTD) Bundled Benefit Plan.

If your church does not have computer access (or you prefer to fill out paper forms), use the Enrollment/Change Form, Church Billing Setup Form, Church Benefit Election Form, or The Hartford Life/Disability Beneficiary Designation Form below. These are fillable PDF forms; you must download to your computer (when the document opens in your browser, right-click and “Save As” to your computer) and fill them out in Adobe Acrobat or the information will not be saved.

Enrollment/Change Form

Participants should use this form to enroll or make changes to their Benefit Plans coverage.

Church Billing Setup Form

Churches that are new to EPC Benefits or existing participating churches that need to update their billing information.

Church Benefit Election Form

This form must be completed by churches enrolling in EPC Benefits for the first time. Changes will be reflected in the online portal only after this form has been completed and submitted.

Life/AD&D/LTD Beneficiary Designation Form

This form must be submitted to the Admin Office for employees who enroll in the Life/AD&D/LTD bundled benefit.


The 2021 Benefits Guide provides a high-level overview of the plans, including eligibility requirements, five medical plan options, two dental options, vision benefits, life and disability benefits, retirement plan, telephonic medical care, and more.


The monthly premium rates for the 2021 Medical, Dental, Vision, Life and AD&D, and Long-Term Disability (LTD) plans are available at This online calculator is a great tool for budgeting and for choosing a plan that fits within your budget.


98point6 provides primary care physician visits, on-demand 24/7, from any location via text messaging using your mobile phone or smart device. Visits with 98Point6 physicians are available to employees and dependents ages 1 and older enrolled in any of BRI’s Medical/Prescription Drug, Dental, Vision, or Life Insurance and Disability benefit plan. Each text-based visit is available at no cost to PPO plan participants, and for all other eligible participants there is only a $5 co-pay.

The EPC has already confirmed Plan participants’ eligibility for 98Point6. Simply download the app on your smart device and register using the name on any of your EPC Benefit Plan insurance cards. You will have immediate access to the service if a medical question or health condition arises.

Vision benefits offer plan participants access to more than 94,000 providers nationwide, including major retail eyecare chains and thousands of private practitioners. Participants are able to receive both lenses and contacts annually, as well as Lasik discounts, retinal screening, and the EyeEssentials discount plan for purchases made beyond normal coverages. The SmartBuyer tool includes guides to help you make informed decisions to both maximize your benefit value and minimize your out-of-pocket costs as you navigate the maze of eyewear options.

BRI offers Life, AD&D, and Long-Term Disability insurance through The Hartford.

Life insurance coverage offers eligible employees a $50,000 Term Life Insurance policy (decreases after the age of 65). Should your death be due to an accident, your beneficiaries would receive an additional AD&D (Accidental Death and Dismemberment) benefit equal to the amount of the life insurance benefit.

Long-Term Disability (LTD) insurance provides partial income replacement should you become disabled and unable to work. If you experience a disability that lasts 90 days or more, the LTD plan will replace 60% of your monthly pre-disability salary up to the plan maximum.

The Amplifon hearing aid program provides discounted hearing aids through more than 5,000 audiologists throughout the country. There are other benefits provided when working through this network.

This benefit is available to EPC employees working 20 hours or more per week.

Churches interested in acquiring additional Life, Accident, and/or Short-Term Disability Insurance for their employees can set up a policy directly with Colonial Life. These benefits are contracted directly with Colonial Life, therefore churches are not restricted to the Open Enrollment time limitations for enrollment.

These voluntary benefits are available to EPC employees working 20 hours or more per week.

The BRI 403(b)(9) Retirement Plan offers you an opportunity to save for retirement with both pre-tax and post-tax dollars. The Plan is managed by Fidelity Investments and offers more than 22 mutual fund investment options in the standard lineup. Additionally, Fidelity BrokerageLink® provides plan participants with the opportunity to select from thousands of mutual funds and other investment options beyond the standard lineup, such as stocks, Exchange-Traded Products (ETPs), CDs, and more.

Your church may contribute to your 403(b)(9) account to help your savings grow. Employer contributions are deposited to your traditional pretax 403(b)(9) plan. Any amount the church contributes to your account is fully vested and yours to keep. Individuals age 50 and older also may make additional “catch up” contributions. Check with your church administrator for more information.

The BRI 403(b)(9) retirement plan ID number is 89560.

Note that enrollment in the 403(b)(9) Retirement Plan—and changes to investment allocations—can occur any time, and is not limited to the Open Enrollment period.
