2022 PLAN INFORMATION
Welcome to EPC Benefit Resources, Inc.! We hope the information you find here will be helpful as you continue in—or are enrolling in—EPC Benefit Plans for the 2022 calendar year. Employees interested in participating in an EPC benefit plan should speak with their church’s benefit administrator about EPC plans that are available to you.
The EPC provides five Medical Plan options to the staffs of EPC churches and ministries. Plans include traditional Platinum, Gold, and Silver Plans, as well as High-Deductible (HDHP) Gold and Bronze Plans with Health Savings Account (HSA) options. All five plans use the same nationwide unrestricted network of hospitals, doctors, medical practitioners, and pharmacies that are used by major national employers and health plans throughout the country. All include 24/7 telemedicine, prescription drug coverage, and wellness programs. Additionally, they provide special assistance programs to support those with chronic conditions, or who encounter medical emergencies or utilize high-cost medications. When you enroll in the EPC Medical Plan, you also are enrolled in the Prescription Drug Plan.
It is important to note that as we come together and share one another’s burdens, the EPC has established its own plans with customized coverages. Your premium dollars will never support procedures that are not consistent with our values as Christians.
The EPC also provides Dental and Vision benefits, as well as Life and Disability Insurance coverages. General information on all EPC benefits can be found in the Benefits Guide below. BENEFITS GUIDE
The 2022 Benefits Guide provides a high-level overview of the plans, including eligibility requirements, medical plan options, dental and vision benefits, life and disability benefits, retirement plan, telephonic medical care, and more.
FOR BENEFIT ADMINISTRATORS
If there are no changes to your staff plan coverage selections for 2022, no action is required. All will automatically stay enrolled in their current benefits.
The 2022 Open Enrollment period begins November 1, 2021, and ends November 30, 2021. All elections made during this annual open enrollment period become effective on January 1, 2022. This is the only window where employees can make changes to their benefit selections unless they experience a qualifying life event at another time during the program year.
For enrolling new employees, or to make changes to your employees’ plan selections, follow the instructions below.
Church administrators can enroll and manage employee benefit coverage options through the EPC Benefits Portal. Administrators should contact the EPC Administration Office at (877) 578-8707 or EPC@cdsadmin.com to set up their user ID and password and for additional information. The EPC Benefits Portal is used to enroll and make changes to the plan options for eligible employees in the:
• Medical/Rx Plans.
• Dental Plans.
• Vision Plan.
• Life/AD&D/Long-Term Disability (LTD) bundled benefit plan.
If your church does not have computer access (or you prefer to fill out paper forms), use the fillable PDF forms below. NOTE: You must download these forms to your computer (when the document opens in your browser, right-click and “Save As” to your computer) and fill them out in Adobe Acrobat or the information will not be saved. Benefit Plans Enrollment and/or Change Form
Use this form to enroll or make changes to your employees’ coverage.
Life/AD&D/LTD Beneficiary Designation Form
This form must be submitted to the Admin Office for employees who enroll in the Life/AD&D/LTD bundled benefit.
Church Benefit Plans Election Form
If your church is enrolling staff in EPC Benefits for the first time, or if you are adding a new benefit for your staff for 2022, you will need to complete this form.
Church Billing Setup Form
If you are new to the EPC benefit plans, use this form to get your church set up through EPC Billing Administration.

403(b)(9) RETIREMENT PLAN
You can enroll staff in the EPC Retirement Plan at any time, it is not limited by a specific enrollment period. For information on how to enroll in the EPC Retirement Plan please refer to the 403(b)(9) Retirement Plan.
