2019 OPEN ENROLLMENT
Welcome to EPC Benefit Resources, Inc. We hope the information you find here will be helpful as you continue in—or want to become enrolled in—the EPC Benefit Plans for the 2019 calendar year.
The 2019 Open Enrollment period for our Plans begins November 1, 2018, and ends November 30, 2018. The elections made during this annual open enrollment period become effective on January 1, 2019. Please work with your staff for a timely submission of benefit selections for 2019, as this is the only window where employees can make changes to their benefit selections, unless they experience a qualifying life event at another time during the program year. Please refer to the Medical Plan document for information on qualifying life event changes.
If there are no changes to your staff plan coverage selections for 2019, no action is required.
For enrolling new employees, or to make changes to your employees’ plan selections, follow the instructions below.
Church administrators can enroll and manage employee benefit coverage options through the EPC Benefits Portal. Administrators should contact the EPC Admin office at 877-578-8707 or EPC@cdsadmin.com to set up their user ID and password and for additional information. The EPC Benefits Portal is used to enroll and make changes to the plan options for eligible employees in the:
• Medical Plans.
• Dental Plans.
• Vision Plan.
• Life/AD&D/Long-Term Disability (LTD) Bundled Benefit Plan.
If your church does not have computer access (or you prefer to fill out paper forms), use these forms:

You must download to your computer (when the document opens in your browser, right-click and “Save As” to your computer) and fill them out in Adobe Acrobat or the information will not be saved. 2019 PREMIUM RATES CALCULATOR
The monthly premium rates for the 2019 Medical, Dental, Vision, Life and AD&D, and Long-Term Disability (LTD) plans are available at www.epc.org/benefits/2019premiumrates. This online calculator is a great tool for budgeting and for choosing a plan that fits within your budget.
New to the calculator this year are premium rates for the Medicare Comprehensive Medical Plan and Missionary Gold Plan.
2019 MEDICAL PLAN
Benefit Resources, Inc., provides medical insurance coverage through Highmark Blue Cross Blue Shield. EPC Medical Plan participants have access to more than 96 percent of hospitals and 95 percent of doctors and specialists contracted through the Blue Cross Network. We offer three PPO plans and two high-deductible medical plan options for you to choose from. When you enroll in the Medical Plan, you will also be enrolled in the Prescription Drug Plan.
The handy side-by-side comparison chart available below shows the deductibles, co-pays, out-of-pocket limits, and other costs for each of the Medical and Prescription Drug plans (Bronze HDHP, Silver, Gold, Gold HDHP, and Platinum), including the differences between in-network and out-of-network providers.

2019 PRESCRIPTION DRUG PLAN
The BRI Prescription Drug Plan is administered by Express Scripts.
Prescriptions dispensed for acute care (short-term) medications and initial fills of maintenance (long-term) medications may be obtained through any retail pharmacy for up to a 30-day supply. Short-term drugs include antibiotics and other medications that you take for short periods of time. Long-term drugs, also called maintenance medications, are those you take on an ongoing basis, such as drugs that treat high blood pressure, cholesterol or chronic diseases. Maintenance medications are only available under the Smart90 program.
For those using Specialty Medications that require focused care and monitoring to maximize effectiveness of treatment, BRI has contracted with Accredo, an Express Scripts specialty pharmacy with more than 25 years of experience in the field. Accredo provides comprehensive local clinical services and 24/7 personalized support and education to patients navigating complex health issues through pharmaceutical therapy.

2019 DENTAL PLAN
BRI offers two dental plans through Principal Dental. The Low Plan provides a lower benefit level for a lower monthly premium. The High Plan provides more comprehensive coverage with a higher annual maximum benefit. Both plans use the same network of dentists. You may visit any dentist; however, you receive the greatest benefits when you visit network dentists.

2019 VISION PLAN
BRI offers a Vision Plan through the EyeMed network of providers. The vision plan covers an exam and lenses every 12 months, and covers eyeglass frames every 24 months.
For the greatest benefits, visit a doctor in the EyeMed network—you can search for a doctor online at www.eyemedvisioncare.com. EyeMed has several networks, so be sure to select the EyeMed “Access” network when searching.

2019 LIFE, AD&D, AND LONG-TERM DISABILITY
BRI offers Life, AD&D, and Long-Term Disability insurance through The Hartford.
Life insurance coverage offers eligible employees a $50,000 Term Life Insurance policy (decreases after the age of 65). Should your death be due to an accident, your beneficiaries would receive an additional AD&D (Accidental Death and Dismemberment) benefit equal to the amount of the life insurance benefit.
Long-Term Disability (LTD) insurance provides partial income replacement should you become disabled and unable to work. If you experience a disability that lasts 90 days or more, the LTD plan will replace 60% of your monthly pre-disability salary up to the plan maximum.

ADDITIONAL LIFE, ACCIDENT, AND SHORT-TERM DISABILITY
Churches interested in acquiring additional Life, Accident, and/or Short-Term Disability Insurance for their employees can set up a policy directly with Colonial Life. These benefits are contracted directly with Colonial Life, therefore churches are not restricted to the Open Enrollment time limitations for enrollment.
These voluntary benefits are available to EPC employees working 20 hours or more per week.

HEARING AID DISCOUNT PROGRAM
Participants enrolled in the EPC Benefit Plans—including the Retirement Plan—can take advantage of the Amplifon hearing aid discount program. This program provides discounted hearing aids through more than 5,000 audiologists throughout the country. There are other benefits provided when working through this network.
This benefit is available to EPC employees working 20 hours or more per week.

403(b)(9) RETIREMENT PLAN
The BRI 403(b)(9) Retirement Plan offers you an opportunity to save for retirement with both pre-tax and post-tax dollars. The Plan is managed by Fidelity Investments and offers more than 22 mutual fund investment options in the standard lineup. Additionally, Fidelity BrokerageLink® provides plan participants with the opportunity to select from thousands of mutual funds and other investment options beyond the standard lineup, such as stocks, Exchange-Traded Products (ETPs), CDs, and more.
Your church may contribute to your 403(b)(9) account to help your savings grow. Employer contributions are deposited to your traditional pretax 403(b)(9) plan. Any amount the church contributes to your account is fully vested and yours to keep. Individuals age 50 and older also may make additional “catch up” contributions. Check with your church administrator for more information.
The BRI 403(b)(9) retirement plan ID number is 89560.
Note that enrollment in the 403(b)(9) Retirement Plan can occur any time, and is not limited to the Open Enrollment period.
