Life, AD&D, and Disability Plans

For the Life Insurance / LTD Plan, a person is eligible for Employee coverage on the first day of employment.

TERM LIFE AND AD&D INSURANCE 

 

As a part of the EPC benefits offerings, the EPC offers eligible employees a $50,000 Term Life Insurance policy (decreases after the age of 65). An Accidental Death and Dismemberment (AD&D) insurance policy is also included and is equal to the amount of the life insurance benefit.

LONG - TERM DISABILITY (LTD) INSURANCE 

 

The EPC offers eligible employees Long-Term Disability Insurance coverage as part of the EPC Benefits Program. This coverage provides 60 percent of qualified earnings (up to $4,000/month) after 90 days of total disability. 

For those enrolled in LTD, Ability Assist Counseling Services are available at no additional cost. These services include emotional or work-life counseling; financial information and resources; and legal support and resources.

QUALIFYING LIFE EVENT/FAMILY CHANGES
A Qualified Life Event is a change in your family status or employment that affects your benefits eligibility. Examples include marriage; birth or adoption; divorce or separation; a dependent child aging out of benefits at age 26; gain or loss of a spouse’s health coverage; or a spouse’s employer offering benefits with a different Open Enrollment period.

If you experience a Qualified Life Event, you must notify the EPC Benefits Administration Office and request benefit changes within 30 days of the event. Click here to download the EPC Benefits Enrollment/Change Form.
