    2. At the end of this form you will attach a one-page paper on the topic, “What is Reformed Theology?” Acceptable file formats: .doc, .docx, .pdf

    3. At the end of this form you will attach a recent photo of yourself. Acceptable file formats: .jpg, .gif, .png, .pdf

    4. Five CWCC-issued written references will be required; at least one of these references will receive a follow-up phone call. Reference forms will be sent to you after we receive your completed application packet. Your references must include a Teaching Elder and a Ruling Elder.
